What should be done if fetal pole and yolk sac are not seen in a first ultrasound?

Patient's Query

Hello doctor,

I am 8 weeks and 5 days pregnant. I went for my first sonography which showed five weeks and one day, and a gestational sac was seen, but no fetal pole and yolk sac. What should I do? Is there any problem? Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I can understand your concern.

To confirm that a pregnancy is viable (normally formed), three things should be seen on an ultrasound scan - gestational sac, fetal pole, and cardiac activity. If you see a gestational sac alone without a fetal pole, there are two possibilities:

1. Pregnancy is still early.

2. Pregnancy was not properly formed and will need to be removed which is called blighted ovum or anembryonic gestation.

In your case, at eight weeks, they should be able to see the gestational sac, fetal pole, and fetal cardiac activity. So, if a gestational sac is only visible, there might be a problem. In my opinion, you should get a transvaginal ultrasound scan done. If it still shows only a gestational sac, the pregnancy is a blighted ovum. If you cannot get a transvaginal ultrasound scan done, wait for two weeks and repeat the transabdominal ultrasound scan.

I hope this helps.

Thank you.

Regards.
