Patient's Query
Hello doctor,
The top of my butt crack has been itching for the past two days. I decided to scratch and a bump appeared. When I woke up this morning I noticed that it was still itchy but the bump was there anymore. I reached the back to feel it, but it felt a little wet as if it popped. I do not know what happened while I was sleeping. Can you please tell me what this is? I have been reading about it online and all that keeps popping up is herpes. Is it right?
Hello,
Welcome to icliniq.com.
I went through the details and photo given by you. (attachment removed to protect patient identity). Though the photo is not clear it appears as excoriated vesicle. You have not mentioned your age and symptoms like pain. It is at the top of your butt crack and the skin appears dry. I do not see any pus. This is unlikely to be due to herpes.
It can be due to irritant dermatitis or dryness causing dermatitis or any insect bite if pain also present. Please clean the area with warm water twice daily. Apply topical Eumosone (Clobetasone) cream twice daily. Take antihistamine like Cetirizine. You can get back if it does not subside or any new lesions develop anywhere else.
Patient's Query
Thank you doctor,
After getting out of the shower the sore came off and it still itches. This is what it looks like (photo attached). Are you sure this is not an STD?
Hi,
Welcome back to icliniq.com.
The photo is clear and it appears like an ulcer. (attachment removed to protect patient identity). If there is unprotected anal intercourse history it can be an STD (sexually transmitted disease). It can be chancroid. But you do not have pain which is against it.
I would treat it with a course of antibiotics like Azithromycin 500 mg twice daily for five days. Clean the area with warm water two to three times daily. Look for fresh lesions. Take antihistamine like Cetirizine for itching. Do not apply any topical for now. If you have a history of unprotected intercourse or with multiple partners, consult your doctor for examination investigation and treatment. Please ask the partner to check for the presence of any STD and also get investigated for STDs. If no inter course history then you can relax. You can get back for any clarification.
